Well, this song is perfect for an anime sheet score, and I'm sure someone has already done it. It *was* requested, though, so I had to come up with a chord version. I know that guitar players might look at the chords and go "HUH?", but I can't chord the song without having to add in all the weird tensions because...it's a piano song. If you're a piano player, I highly recommend getting an accurate sheet version of this to make learning easier (unless you know how to play the chords I wrote), but for guitarists...try your best!
Let me also state that these are the best chord approximations I can give for a fully piano song, without actually writing the score down for you. If you've seen the sheet music, then you might think that one or two chords aren't exactly the correct description for that certain bit. Guess what? I'm sure they aren't - but there's only so much you can write without making them look like Gbsus2(add9, b13) or etc.
Notes
{1} Because of the melody note falling on a Gb, the second Fm7 chord is *actually* an Fm7(b9), but since all the pianist did was play the Fm7 chord, that's what I wrote down.
{2} It's actually more like a Gbmaj9,13. But you didn't need to know that ^_^
