Since this notation was a request, the file that I got was incomplete so until I can obtain a complete version of this song, I can only get the chords until the end of the instrumental part.
Also note that I corrected some wrong romanization in the lyrics, so you can say it's not taken exactly from animeart.com's lyrics.
Notes
{1} The Bdim that I wrote sounds to me like a Bdim, but could be a G/B or even something else.
{2} I'm guessing the Gm/Bb. I know the bass is a Bb though.
{3} The key changes to Eb for the instrumental right here at the Ab/Bb
{4} At the end of the instrumental, the electric guitar holds a C, but in the background the keyboard plays the C F/C G/C progression twice. The key also changes back to C at the first C on this line.
